Sclerosing Polycystic Adenoma in Parotid Gland: Case Report for Palliative Treatment with Rare Clinical Presentation
Abstract
Sclerosing polycystic adenoma is a rare benign tumor of the salivary glands. Less than 100 cases have been reported worldwide. The genetic alterations found in salivary duct carcinoma suggest a close relationship to apocrine intraductal carcinoma due to the presence of non-neoplastic surrounding myoepithelial cells. We are presenting the case of A 79-year-old Saudi man with a known history of diabetes and hypertension, presented with discomfort left parotid mass and pain for two years progressive in size accompanied by hearing loss, and blood coming out. In the local examination, the mass appeared large well-circumscribed cystic mass, grayish to pink, ovoid in shape, smooth in surface, firm in consistency, non-tender freely movable, 10 by 15 cm in size, and with multiple skin erythematous discoloration, skin scratches and oozing crusty ulcer. CT of the head and neck revealed a mass in the superficial lobe of the left superficial lobe the parotid gland. Fine needle aspiration and core needle biopsy were done. Sclerosing polycystic adenoma diagnosis was confirmed. The patient was referred to palliative treatment since he refused surgical intervention. This case report study was the first case presenting with this unusual presentation which was, the oldest case report of Sclerosing polycystic adenosis in the parotid gland, skin lesions, ulcers, or oozing blood associated with 15 cm tumor size mass which was the biggest record tumor.
