Overview of breast cancer screening program in Indonesia using clinical breast examination and ultrasonography testing

Abstract

Breast cancer was the most common cancer among females in Indonesia. Launched in 2008,the breast cancer screening program was conducted in all districts across Indonesia. This study aimed to overview the program of breast cancer screening using clinical breast examination and Ultrasonography (USG) examination. This was a cross-sectional study using secondary data from the Ministry of Health, 2024. The population of this study was all districts/cities in Indonesia conducting the screening, consisting of 4,757,732 for breast cancer screening and 603,408 for USG testing. Using the total sampling method, 508 districts/cities were included as sample with inclusion criteria having complete data. Descriptive analysis, comparative analysis using Mann-Whitney test, and correlative analysis using Spearman correlation test were performed in this study using district as analysis unit. Results of the clinical beast examination showed that there was 4.46 per 1000 (21,197 cases) lumps in the breast, and 0.16 per 1000 (738 cases) suspected of breast cancer, Meanwhile, USG test resulted in 1.11 per 1000 (672 cases) simple cysts and 1.94 per 1000 (1169 cases) non-simple cyst. There was no difference of lump in the breast, suspected breast cancer, and simple cyst between West and East of Indonesia, but there was a difference for non-simple cyst. There was also no difference of lumps in the breast, suspected breast cancer, simple cysts between, and non-simple cyst between district and city. There was a positive correlation between overweight, obesity, central obesity, dyslipidemia and lump in the breast, but there was no positive correlation between these risk factors and suspected breast cancer, simple cyst, and non-simple cyst.

Biography

Mugi Wahidin is a Doctor of Public Health, graduated from Universitas Indonesia in 2023. Since 2006, he became an epidemiologist focusing on non-communicable disease (NCD) till 2017. Then, he became a researcher at the National Institute of Health Research and Development, Ministry of Health from 2017-2022. Now, he has been a senior researcher at the National Research and Innovation Agency since 2022. He has been conducting research focusing on NCD, including cancer. Some of which are cervical and breast cancer screening program (2020), oral contraceptive and breast cancer risk (2018), and population-based cancer registration in Indonesia (2012).
